Register on the Patient Portal

Prior to your first visit, register on the Rose Wellness Patient Portal. Please fill in your intake forms at least 2 business days before your scheduled appointment. The intake forms take approximately 30 minutes to complete. These help our practitioners better understand your health history and lead to a productive initial consultation. If you need assistance, please call us and one of our helpful staff members will assist you

Medical Records

Scan and upload your previous medical records (recent lab tests, radiology reports, or other medical reports) through our secure patient portal or fax them to our office prior to your first visit. Please call us if you need any help with this.

What To Bring At Your First Visit

We encourage you to bring any nutritional supplements that you take regularly including any vitamins, minerals or herbal supplements. Our practitioners will want to verify the ingredients and strengths of each supplement to get a better idea about the nutrients you are receiving.

What To Expect At Your First Visit

For your initial visit with your integrative medicine practitioner, plan on spending 1 1/2 to 2 hours. 60-90 minutes of this time will be devoted to a thorough consultation. The other 30 minutes will be used for checking in and out and any lab tests that the practitioner may prescribe

During the consultation, your practitioner will review your medical records and discuss any previous diagnoses, treatments and medications you currently take. Information on the intake forms will be discussed in detail and diagnostic testing may be ordered to help identify the root cause of your condition. The physician will develop an individualized treatment plan based on the findings using a functional medicine approach.


Patient Care and Education

A significant portion of your visit will consist of patient education. Your practitioner will address and answer your health questions. We help you understand your medical conditions and the importance of finding the root causes.

Patient Practitioner Relationship

To get the most out of your consultation, it helps to be open about any symptoms you are experiencing, medications you are taking, health history and lifestyle choices. This information helps our practitioners provide you with the best care you need. Please be assured that all information you provide is confidential.

Follow Up Visits

Most follow up appointments last about 30 to 45 minutes. During this time, your practitioner will evaluate your progress and continue to follow up on the treatment plan designed for you. Any adjustments and “fine tuning” may be made to your program if indicated. The physician may also order additional lab tests to track any functional imbalances or deficiencies.

Many patients find that after their initial consultation, questions arise, or they may have forgotten to tell the doctor about some past medical history. At the follow up appointment, the physician will give ample time to answer any questions and take down any further medical information. Finally, if any changes need to be made, the doctor will discuss each change and make sure you completely understand each change.

Insurance Free Practice

Focused on you, not insurers

We believe in empowering you to have the freedom of choice to determine the best treatment options for your medical needs and avoid constraints placed by insurance companies.

We are an “Insurance Free” practice. However, we want to help you receive any insurance benefits you may be entitled to. After each visit, our staff will provide you with a “superbill” with the appropriate diagnostic and procedure codes. The “superbill” enables you to submit a reimbursement claim to your insurance company for reimbursements. Our physicians at Rose Wellness are considered “out of network.”

Most insurance companies offer some degree of reimbursement; however, you are encouraged to check with your insurance company about reimbursement prior to your appointment. Furthermore, if your insurance company needs further information, feel free to contact our office and our helpful staff will try to assist you. Please note that if you are a Medicare beneficiary, you will not be entitled to any reimbursement from Medicare. All costs for services rendered will be your sole responsibility.
