Prior To Your First Visit
Register on the Patient Portal
Prior to your first visit, register on the Rose Wellness Patient Portal. Please fill in your intake forms at least 2 days before your scheduled appointment. The intake forms should take approximately 30 minutes to complete. These help our practitioners better understand your health history and lead to a productive initial consultation.
If you have any questions or need assistance regarding your intake forms, please contact our office and one of our helpful staff members will assist you.
Send Us Your Medical Records
Please collect your pertinent or recent lab tests, radiology reports, or other medical reports which you feel may be helpful in evaluation of your overall health. You may request these medical records from your family physician, any specialists and/or hospitals.
Scan and email all your records through our secure patient portal or fax them to our office prior to your first visit. Please call us if you need any help with this.
What To Bring At Your First Visit
We encourage you to bring any nutritional supplements that you take regularly including any vitamins, minerals or herbal supplements. Our practitioners will want to verify the ingredients and strengths of each supplement to get a better idea about the nutrients you are receiving.
Patient Doctor Relationship
Your consultation will only be as good as the information you are willing to share. In order to get the most out of your consultation, you must be open and honest about any symptoms you are experiencing, medications you are taking, health history and lifestyle choices. This information helps our doctors provide you with the best care you need. Please be assured that all information you provide is confidential.
Your Initial Consultation
For your initial visit with your integrative medicine physician, plan on spending 1 1/2 to 2 hours. 60-90 minutes of this time will be devoted to a thorough consultation. The other 30 minutes will be used for checking in and out and any lab tests that the doctor may order.
During the consultation, your doctor will review all of your medical records and discuss any previous diagnoses, treatments and medications you currently take. All of the information on the intake forms will be discussed in great detail and diagnostic testing may be ordered to help identify the root cause of your condition. Then, the doctor will develop an individualized treatment plan based on the findings using a functional medicine approach.
A significant portion of your visit will consist of patient education. Your practitioner will address and answer all of your health questions. At Rose Wellness Center, we believe it is important for you to understand your body and your health. We spend time making sure you understand your medical conditions and the importance of finding the root cause of the condition.

Follow Up Visits
Most follow up appointments last about 30 to 45 minutes. During this time, your doctor will evaluate your progress and continue to follow up on the treatment plan designed for you. Any adjustments and “fine tuning” may be made to your program if indicated. The physician may also order additional lab tests to track any functional imbalances or deficiencies.
Many patients find that after their initial consultation, questions arise, or they may have forgotten to tell the doctor about some past medical history. At the follow up appointment, the physician will give ample time to answer any questions and take down any further medical information. Finally, if any changes need to be made, the doctor will discuss each change and make sure you completely understand each change.
